For eligible cash patients, Eisai will pay up to $60 per prescription for a maximum of $720 per year . Commercially insured and cash patients could have additional financial responsibility for any amounts over Eisai's maximum liability.
How old do you have to be to get a prescription card?
You must be 18 years or older to use the card for yourself or a minor. Patients and pharmacies are responsible for disclosing to insurance carriers the redemption and value of the card and complying with any other conditions imposed by insurance carriers, third-party payers, and applicable law on the redemption.
Is a fycoma card valid for prescriptions?
