What is Horizon by your side?

We will be by your side. It can make all the difference to have someone in your corner: a dedicated team member who will support you throughout your treatment. The Horizon By Your Side team is committed to improving lives with services and support for you and your family.

What is a PAL in Horizon?

Once enrolled, you will be paired with a Patient Access Liaison (PAL). Your PAL provides you with dedicated, one-on-one support. They work directly with you to answer non-medical, logistical questions and provide support once you have been prescribed a Horizon medicine and you have completed enrollment.

What does a patient access liaison do?

Once you and your healthcare team agree on a plan that works best for you, your Patient Access Liaison or Clinical Nurse Educator will talk to you about the insurance approval process and what your potential out-of-pocket treatment costs may be

Can a nurse educator submit a patient enrollment form?

They will have the enrollment form and can submit it for you after you’ve signed it. Once you’ve signed the Patient Enrollment Form, your Patient Access Liaison or Clinical Nurse Educator will take the first step by calling you to introduce themselves and help you with non-medical or logistical questions.

How to enroll in Horizon by your side?

Patient enrollment in Horizon By Your Side is easy. Download, complete with your patient, and submit the Patient Enrollment Form below. There is a specific HIPAA release section on the form required in order to initiate enrollment for your patient in Horizon By Your Side. Have your healthcare team work with your patient to finalize the form.

What does Horizon work closely with?

Works closely with Specialty Pharmacies to ensure all required information is received for the timely delivery of Horizon products to your patients

What happens when you enroll in Horizon By Your Side?

Once your patient is enrolled in the program, you will receive a comprehensive summary of your patient’s benefits from the Horizon By Your Side team. We will be there to provide support when your patient needs it throughout their treatment journey.

The Site of Care Team establishes business-to-business relationships with sites of care and expands the network of infusion center options. The team educates on coding, billing, and payer access and provides product in-servicing.

What is the assistance fund?

The Assistance Fund is an independent charitable patient assistance organization that provides support for adults and children with rare and chronic diseases. The organization may help provide families with cost and travel assistance.

What is the Patient Advocate Foundation?

The Patient Advocate Foundation provides support and assistance to patients with chronic, debilitating diseases to help access care and treatment recommended by their doctor. Case managers will help patients understand roadblocks to access to care and affordability to Americans with chronic, life-threatening, and debilitating illnesses.
