Who is eligible for the insupport® copay assistance program?
The INSUPPORT® Copay Assistance Program is valid ONLY for patients with private insurance who are prescribed PERSERIS for on-label use.
How do I enroll a patient in the copay assistance program for perseris?
Note that for patients enrolled in "Benefit Coverage Information", INSUPPORT® will automatically determine eligibility and enroll an eligible patient in the Copay Assistance Program for PERSERIS. You may enroll an eligible patient online by going to the INSUPPORT® Portal located at www.insupportportal.com.
Are patients with government insurance eligible for the copay assistance program?
Patients with government insurance are not eligible for the Copay Assistance Program, including, but not limited to, Medicare, Medicaid, Medigap, VA, DoD, TRICARE, CHAMPVA, or any other federally or state-funded government-assisted program. Other restrictions apply.

Is Perseris covered by insurance?
Perseris is currently not covered by any Medicare insurance plans. Without coverage, you may have to pay the full cash price of $2,638.95. Save on this total by using a SingleCare savings card.
What is Insupport?
INSUPPORT was created to provide information aimed at helping appropriate eligible patients with the process of obtaining SUBLOCADE. INSUPPORT can provide information about your health insurance benefits for SUBLOCADE and other potential financial assistance options that you may qualify for.
How often does Sublocade release?
SUBLOCADE comes in 2 doses: 300 mg and 100 mg. You'll receive a dose once a month (with at least 26 days between doses). Your treatment provider will start with 300 mg to help the medication reach an effective level in your body.

How to enroll in Insupport?
There are two ways to enroll a patient in the Program: 1 Online Enrollment#N#You may enroll an eligible patient online by going to the INSUPPORT® Portal located at www.insupportportal.com. If the patient is not in the office at the time of enrollment, you may request patient authorization from the Portal and the patient can complete and submit the form to INSUPPORT® via DocuSign. Once the patient authorization is received, INSUPPORT® can process the request. Please ensure both provider and patient read the Terms and Conditions for the INSUPPORT® Copay Assistance Program and provide the required electronic signature and date. 2 Patient Enrollment Form#N#Download the Patient Enrollment Form using the link below.#N#Select "Copay Assistance Program for PERSERIS" and complete the required steps outlined on page 1. Please ensure that the signature and date are provided on both the Provider Attestation and Patient Authorizations.#N#Fax the completed form directly to INSUPPORT® at 1-833-404-4897.
Can you get copay assistance with Medicare?

Does Insupport automatically determine eligibility?
Note that for patients enrolled in "Benefit Coverage Information", INSUPPORT® will automatically determine eligibility and enroll an eligible patient in the Copay Assistance Program for PERSERIS.
Does Insupport provide copay ID?
INSUPPORT® will provide the enrolling healthcare provider with the patient's Copay Member ID for submission of copay claims to INSUPPORT®, as well as copay claim submission instructions (this information is provided immediately if using web enrollment). A Copay Member ID card will be provided to the patient in his/her welcome letter upon enrollment in the program.
Copay Assistance Program
INSUPPORT® offers a Copay Assistance Program designed to help eligible patients with the out-of-pocket costs for PERSERIS. You may work with your healthcare provider who may determine your eligibility for the Program, and enroll you if eligible, during an in-office appointment.
Alternate Funding Information
If you do not have insurance coverage for PERSERIS, INSUPPORT® can help you learn about other potential coverage resources that you may qualify for using the INSUPPORT® Patient Enrollment Form. If applicable, you may receive this information as part of a request for benefit coverage information for your current or new site of care.
INDICATION
SUBOXONE (buprenorphine and naloxone) Sublingual Film ® (CIII) is a prescription medicine used to treat opioid addiction in adults and is part of a complete treatment program that also includes counseling and behavioral therapy.
IMPORTANT SAFETY INFORMATION
Keep SUBOXONE Sublingual Film in a secure place out of sight and reach of children, and in a location not accessible by others, including visitors to the home. Accidental use by a child is a medical emergency and can result in death. If a child accidentally takes SUBOXONE Sublingual Film, get emergency help or call 911 right away.
