How do I enroll a patient in the copay assistance program for perseris?
Note that for patients enrolled in "Benefit Coverage Information", INSUPPORT® will automatically determine eligibility and enroll an eligible patient in the Copay Assistance Program for PERSERIS. You may enroll an eligible patient online by going to the INSUPPORT® Portal located at www.insupportportal.com.

How much does perseris cost for extended-release suspension?
Some of your patients may be eligible for savings on PERSERIS® (risperidone) for extended-release injectable suspension. Eligible patients may pay as little as $5 per injection of PERSERIS. Restrictions apply. The Program benefit is valid for the out-of-pocket cost for PERSERIS only.
Who is eligible for the insupport® copay assistance program?
The INSUPPORT® Copay Assistance Program is valid ONLY for patients with private insurance who are prescribed PERSERIS for on-label use.
Can you get copay assistance with Medicare?
Patients with government insurance are not eligible for the Copay Assistance Program, including, but not limited to, Medicare, Medicaid, Medigap, VA, DoD, TRICARE, CHAMPVA, or any other federally or state-funded government-assisted program. Other restrictions apply.
Does Insupport provide copay ID?
INSUPPORT® will provide the enrolling healthcare provider with the patient's Copay Member ID for submission of copay claims to INSUPPORT®, as well as copay claim submission instructions (this information is provided immediately if using web enrollment). A Copay Member ID card will be provided to the patient in his/her welcome letter upon enrollment in the program.
How long is the Perris loan?
Should the business exit the City of Perris during the two years, the loan repayment will be due to the City at a 1% fixed interest rate, with a two-year repayment term
How long is a Perris business loan forgiven?
Loans will be 100% forgivable, provided the business remains in good standing with the City of Perris for at least two years following the disbursement of funds. 50%, to be forgiven one year following loan issuance, and the remaining 50%, to be forgiven two years following loan issuance
