New Patients

If you are a new patient, please arrive 15 minutes before your scheduled appointment to complete any paperwork needed. Please bring the following to your appointment: insurance card, driver’s license, any completed forms, form of payment for any copay, and a list of all your medications.

Payments

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, check, debit and all major credit cards.

Cancellations & Rescheduling

We strive to remain on schedule throughout the day, though emergencies can arise. To better serve our patients, we ask that you call at least 24 hours in advance to cancel or reschedule any appointment.
