How to enroll in Vertex GPS?
Download the enrollment form. The enrollment form is an easy way to help your patients become part of Vertex GPS. Simply follow the steps below. Download the form. by hand. What can a patient support specialist send?
Your patient's Patient Support Specialist can send a summary of important information in the Enrolled Patient Report to help assess treatment adherence.
What to do after enrolling a patient?
After patient enrollment, you can notify the patient or caregiver that a Patient Support Specialist will be reaching out soon.
How many pages to fill out a patient support form?
Be sure to fill out all 3 pages of the form with your healthcare provider. You'll also provide a phone number and the best time your Patient Support Specialist can reach you for a welcome call.
What does a patient support specialist do?
