How do I apply for the Cancer Patient Assistance Program?

To access the Cancer Patient Assistance Program application form please CLICK HERE. As often as possible, we try to arrange for vendors to bill LRCP directly to reduce the patients' out of pocket expenses. When submitting to be reimbursed for previously paid expenses please provide original receipts, not older than 6 months.

How do I donate to the Patient Assistance Program?

To donate to one of the funds that supports the Patient Assistance Program please click here. Funding is available for emergency, short-term situations when funding from other sources and services is not available. Expenses incurred within 6 months of the date of the application will be considered.

How to apply for Lakes Region Scholarship?

Only one application is necessary to apply for all the scholarship funds administered by Lakes Region Scholarship Foundation. Students may download the application, appraisal form and instructions here. Under the section “For Applicants”. Applications may also be obtained by calling 527-3533 or emailing.

Each year the New Hampshire Charitable Foundation distributes more than $4 million in grant aid and loans. They administer more than 350 separate funds.

What is Project Renew Grant?

The goal of Project Renew is to provide monetary grants for tuition to assist single parents and displaced homemakers who are matriculated in a Perkins approved career or technical degree program or certificate program. The Perkins approved program at LRCC are all degree and certificate programs of the college EXCEPT Liberal Arts, General Studies, Fine Arts, and Teacher Preparation. Liberal Arts, General Studies, Fine Arts, and Teacher Preparations students cannot apply for the Project Renew Grant.

What is a scholarship for LNA?

Scholarships are intended for any of the following: Individuals upgrading their job skills or acquiring skills for job entry. (For example, but not limited to – LNA programs, recertification programs, cosmetology programs.) Candidates enrolled for a minimum of three college credits at any accredited facility.

The Patient Assistance Fund

This unique fund has been established to cover the costs of eye surgeries for disenfranchised members of our community who would not otherwise have access to this level of care. PVF’s long history of caring for vulnerable populations has come a long way with the opening of Pacific Vision Surgery Center in November 2020.

How many children have been helped by the Assistance Fund?

Since its founding in 2009, The Assistance Fund has helped nearly 135,000 children and adults access the treatment they need to stay healthy or manage a life-threatening, chronic, or rare disease. Learn more about how we serve our patients.

How much does the NOCC fund cover?

The NOCC Treatment Fund offers a $1,000 grant, once per year, to eligible patients diagnosed with ovarian cancer for non-medical expenses related to treatment and care. The fund will cover transportation to and from treatment/doctor, rent/housing assistance, utilities/bills, food or groceries, child/elder care, and house cleaning services. To be eligible, the patient must meet income guidelines and provide diagnosis documentation to finalize the application. Review the program disclaimer for this fund here.

What is the Patient Advocate Foundation?

This independent division of Patient Advocate Foundation provides small grants to patients who meet financial and medical criteria. Grants are provided on first-come first served basis and are distributed until funds are depleted. Qualifications and processes for each fund may differ based on fund requirements.

Can PAF be used to process applications?

Once a fund becomes fully allocated, PAF is unable to process applications that are pending or accept new applications at that time. Should additional funding become available in the future, it will be necessary for patients to re-apply at that time.
